Performs duties and functions as a CT and Radiology Technologist as assigned. This position requires providing care/service to a critically through sub-acutely ill pediatric through geriatric patient population in a manner that demonstrates an understanding of the functional/developmental age of the individual served. Assesses, plans, implements, and evaluates the individualized occupational therapy care provided to a patient. Responsible for the coordination of care of all assigned patients. Schedule: Full Time Shift : Monday-Friday, 8am – 5pm, flexible schedule Location: We have need for OTs to perform home visits in one or more of the following team areas: Team Green: South Orange County and Osceola County areasThe role you'll contribute: The Home Health Occupational Therapist (OT), under the general supervision of the Care Manager, provides occupational therapy to assigned patients and directs therapy assistants and Home Health Aides to provide quality care in accordance with physician orders under a plan of care. Assesses, plans, implements, evaluates total patient care and coordinates patient/family teaching. Assures compliance with therapy standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. May be on call for after-hours work. Coordinates and directs the home care patient's care based on individual patient needs. The value you'll bring to the team: Performs an evaluation, assessing function using a method which objectively measures activities of daily living such as, but not limited to, eating, swallowing, bathing, dressing, toileting, walking, climbing stairs, using assistive devices and mental and cognitive factors, documenting the measurement results in the clinical record.Administers treatments as discussed in the plan of care involving application of physical agents, and manual techniques to improve and maintain function.
